Case Management helps people living with HIV navigate the care and support systems available in the West Central Texas Area. Case managers are specially trained to respond to the needs of people impacted by HIV/AIDS and help plan for long term financial, emotional and physical needs. They help guide you to accessing important services, support, education and staying involved in healthcare.
Case managers work closely with their clients, medical providers as well as partners and loved ones (when appropriate) to develop and implement a plan of care, to coordinate medical resources, communicate health care needs, and monitor each client’s progress. In addition to meeting the health needs of the clients, Medical Case Managers are the clients strongest advocate when working “the system” to identify additional resources and support needed to meet the human services needs.
Case Management is available to clients of BCAR.
