Case Management

Case Management  is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the recommended treatment options and services required to meet the client’s health and human service needs. It is characterized by advocacy, communication, and resource management. Case Management involves rigorous assessment, care planning, and care coordination to include skills and knowledge building with the goal of independence for each client. Case managers work closely with their clients, medical providers as well as partners and loved ones (when appropriate) to develop and implement a plan of care, to coordinate medical resources, communicate health care needs, and monitor each client’s progress. In addition to meeting the health needs of the clients, Medical Case Managers are the clients strongest advocate when working “the system” to identify additional resources and support needed to meet the human services needs.
