  • determine your eligibility for those services
  • determine any outside services that could benefit you, and initiate any referral process those services require

Please bring as  many  of the following   documents , if you don't have all the required, our Care Coordinators will assist you in completing the list:

BCAR offers a variety of services to our clients.    Our helpful staff are here to determine what help YOU need, what help you’re eligible for, and what outside services we can refer you to for assistance that we don’t provide.
Once you are admitted to service, you and your care coordinator will determine if you need medical case management, non medical case management or no case management.
If case management is determined, you will  complete additional assessments and screenings with your case manager to determine what  services best suit your needs.
If you wish to use our   clinic for HIV care, an appointment will be set up at this time with our provider.
